Commonwealth Home Support Program (CHSP)

CHSP, we offer entry-level aged care services that help people remain at home and engaged with their community.

Our home support includes domestic assistance, personal care, shopping, transport, and various social groups. The transport team provides reliable door-to-door services for medical appointments, shopping trips, and social activities.

Social groups offer opportunities to stay active and connected, with activities like walking groups, coffee catch-ups, day trips, and more.

Care Finder

Our Care Finder service provides intensive support for older people who need help navigating My Aged Care or connecting with other community services.

The program is targeted at people facing barriers such as limited literacy, lack of informal support, or complex circumstances. We work closely with each person to ensure they receive the care and services they need.
